I hereby give Rhythmlink International, LLC and its owners, employees, agents, parent companies, subsidiaries, successors and assigns (collectively, “Rhythmlink International”) the absolute right and unrestricted permission to copy, exhibit, publish or distribute my testimonial statement written above for purposes of publicizing and promoting Rhythmlink International’s products and services or for any other lawful purpose. I authorize Rhythmlink International to use my name, the name of my company and the statement itself. My statement may be used in printed publications, multimedia presentations, on Websites or in any other distribution media or marketing materials.

I waive any right to compensation and agree to make no claim for compensation of any kind or any other monetary claim against Rhythmlink International related to its use of my statement.  In addition, I waive any right to inspect or approve the finished product, including written copy, in which my statement is used.

I hereby hold harmless and release Rhythmlink International and any and all entities and individuals affiliated with Rhythmlink International from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate has or may have by reason of any use of my statement.

I certify that I have carefully read this agreement and that my consent and release as stated and selecting submit are done voluntarily and under no duress. I also certify that I am at least 18 years of age and am competent to execute this agreement.
